For the health and safety of our students, as well as to fulfill state requirements, several information and authorization forms are required as part of our registration process. Required forms vary depending on which session/sessions your child will be attending.
Some of these forms may overlap if your child is attending multiple sessions at DAS. We only need one copy of each form, thanks.

Please Note: We cannot accept any forms before June 1, 2015
